Transition-aged youth (TAY) with intellectual and developmental disabilities (IDD) and their families often face big challenges when moving from child to adult services. This transition involves navigating complex systems—Education, Social Services, and Health—which can be overwhelming for families and care providers. This session will share findings from a five-year study on the experiences of families in a pilot transition clinic, highlighting tools like the SHARE transfer tool and crisis screeners that can help make transitions smoother. Attendees will also hear firsthand perspectives from family caregivers, offering a deeper understanding of the transition journey.

- Learn to use the SHARE transfer tool and crisis screeners to support families during transitions.
- Identify systemic barriers and develop advocacy skills to better support families, especially in times of crisis.
