Health Monitoring: Collaborating with Patients and Their Support Networks

People with developmental disabilities (DD) and their support networks, including family members and developmental service professionals, can work together with healthcare providers to enhance health monitoring, improve communication, and support patient autonomy. This session will use lived-experience insights to address pain and distress as an example of collaborative approaches in health monitoring. By integrating available practice tools, we can improve the exchange of health-related information, foster patient-centered care, and create a more inclusive healthcare experience.

After this session, you will be able to:

  1. Use health monitoring guidelines and tools to enhance health information exchange and care outcomes.
  2. Apply strategies to identify health contributors in complex cases alongside patients and their support networks.
  3. Address pain and distress through team-based health monitoring, using innovative methods and environmental adaptations.
